The Court, having jurisdiction of the parties and the subject matter, considered the Petition
for Modification of Custody filed by ______________, heard the evidence, and being otherwise
fully advised in the premises, finds that it should be granted.
IT IS HEREBY ORDERED that the order entered on ______________, 20 ___ regarding
custody is hereby modified and the court finds:
1.The custody judgment was made on _________________; and that there has been a
substantial change in the circumstances of the parties (and of the child) since the entry of the
