The decree must include the name of the judge or magistrate who heard the case, if applicable DR47(A)(1)(b). The decree must include the words AFinal Appealable Order@ in the caption of the document DR47(A)(1)(c). The decree must include the names, addresses, social security numbers and dates of birth of all parties DR47(A)(1)(d). The decree must include the date of hearing DR47(A)(2). The decree must contain the attorney(s)= signatures, addresses, phone numbers and Supreme Court Registration Numbers, or both parties= signatures - if pro se. (Civil Rule 11) The decree must include a statement of the amount of arrears due and the date the arrears are owed through, if applicable. The dissolution which was heard by a magistrate must include a waiver of findings of fact and waiver of the 14 day objection period, unless form DR47(C). The decree which was heard by a magistrate must contain a signature line for the magistrate DR11(B). Q 10. Q 11. The decree must be accompanied by a Form DR 201, if income withholding or health insurance orders to issue DR9. The decree must be accompanied by the right number of copies ­ without children, original plus three (3) copies and final decrees with children, original plus five (5) copies DR12.
