this cause came before the Court and
the Court finds that the Guardian was appointed on said date for purposes of pursuing an Application to
Settle a Minor’s Claim, that the Application was granted and that after the payment of fees, expenses and
distributions authorized by the Entry Approving Settlement of Minor’s Claim, the net estate of the Ward is
Ten Thousand Dollars or less,
IT IS THEREFORE ORDERED that the Guardianship be and it is hereby terminated and the Guardian
discharged with no further accounting other than the Report of Distribution previously ordered by the Court
in the Entry Minor’s Claim (form 22.4).
