ATHLETE AGENT BOND FORM Know all men by these presents, that we ____________________________________ (Name of Athlete Agent) of ______________________________________________________ (Address, City, State, Zip) as principal, and ________________________________ (Bonding Co. ­ Surety), a corporation organized and existing under the laws of the State of __________________ and authorized to do business in the State of Oklahoma as surety, are held and firmly bound unto the State of Oklahoma hereinafter referred to as obligee, in the penal sum of Fifty Thousand and no/100 __________________ Dollars, lawful money of the United States of America, to the payment of which sum, well and truly to be made, we bind ourselves, our executors, administrators, successors and assigns, firmly by these presents. The bond shall be maintained for not less than two years after the date the athlete agent's certificate of registration expires, or is revoked, surrendered or cancelled. The bond shall be conditioned upon the following: 1) compliance with the Uniform Athlete Agents Act, 70 O.S. §821.81, et seq., as presently worded and as it may hereafter be amended to read, 2) the payment of all sums due a person at the time the sums are due and payable, and 3) the payment of damages suffered by any person as a result of intentional or unintentional misstatements, misrepresentation, fraud, deceit or unlawful or negligent acts or omissions of the principal or principal's representative or employee, while acting as an athlete agent. If the principal shall faithfully comply with all provisions of 70 O.S. §821.81, et seq., throughout the registration period, then this obligation shall be void. If, however, the principal does not comply in all respects with such laws, rule, and regulations identified in this paragraph, this bond will remain in full force and effect. This bond shall not be subject to cancellation by either principal or surety unless written notice of intent to cancel is forwarded to the Oklahoma Secretary of State, not later than the 60th day before the date of cancellation. If the cancellation is at the request of surety, surety shall also provide principal with written notification at least 60 days prior to the effective date of cancellation.
