IN THE SUPERIOR COURT OF ARIZONA YAVAPAI COUNTY Regarding the matter of Case # 1300DO Petitioner and ORDER AND NOTICE TO ATTEND PARENT EDUCATION PROGRAM CLASS Pursuant to A.R.S. §25-352 Respondent THIS IS AN OFFICIAL COURT ORDER. If you fail to obey this Order, the Court may find you in Contempt of Court. Unless the Court excuses your participation, if you fail to complete the educational program as ordered, the Court may deny relief you request, hold you in contempt of Court or impose any other sanctions reasonable including fines up to $300.00. See A.R.S. §25-353. In addition, mediation has a greater likelihood of success with parents who have completed the Parent Education Program. THE COURT FINDS: This case involves minor child(ren) and is an action for either Dissolution of Marriage, Legal Separation or Paternity with Legal Decision Making, Parenting Time and/or Child Support. THE COURT ORDERS: 1. ATTENDANCE: Both PETITIONER and RESPONDENT must attend and complete the Parent Education Program class. 2. TIME LIMIT: Within 20 days from the date of this Order, you must register for the program. RESPONDENT must register for the program within 20 days of being served with this Order and complete the course even if not filing a Response to the Petition. CLASS FEE: If you are the PETITIONER, you must pay the registration fee at the time of filing the Petition. If you are the RESPONDENT, you must pay the registration fee at the time of filing the Response or to the Clerk of the Superior Court before attending the class, if not filing a Response. If you cannot afford to pay the fee, you may request a deferral or waiver of the fee. PARENTS OUTSIDE THE STATE OF ARIZONA: If either parent lives outside the State of Arizona, that parent will be responsible for attending a comparable six-hour Parent Education Program in his/her community, obtaining Court approval for the alternate class and filing proof of attendance with this Court. REGISTRATION FOR THE CLASS AND PAYMENT OF THE FEE DOES NOT CONSTITUTE AN "APPEARANCE" IN THIS ACTION. 3. 4. 5.
