Please take notice that the undersigned has filed a motion with the Bankruptcy Court that the Court certify
fact(s) to the District Court pursuant to 11 U.S.C. § 110(i). A copy of that motion is attached hereto.
You have 20 days from the date of mailing of this notice to file with the Court (and serve on the undersigned)
a written objection to the certification of these facts. The objection must be under penalty of perjury and
must set forth sufficient facts upon which the Court can find that you have not violated the provisions of 11
U.S.C. § 110.
If you fail to timely respond or do not set forth sufficient evidence in compliance with § 110, the Court will
certify this matter to the District Court for imposition of civil fines as set forth in § 110. Should the debtor,
a creditor, or the trustee file a motion on behalf of the debtor with the District Court, you may also be liable
for damages, attorney's fees and costs. You are urged to review the provisions of 11 U.S.C. § 110 and to
consult with an attorney. If you file a timely response, a hearing will be scheduled by the moving party.
However the Court, in its discretion, may determine that no hearing will be held and may rule solely on the
written motion and your response. You are advised to review General Order #95-03 Re: Bankruptcy Petition
Preparers.
