assistance to evaluate the effectiveness of its ADR Program. Please complete this survey and return it to
the ADR Program Office, U.S. Courthouse, 312 N. Spring Street, Room 909, Los Angeles, California
90012. You may also fax this questionnaire to 213-894-5084, or return by e-mail to
