CM/ECF PRO SE LITIGANT ELECTRONIC SERVICE INFORMATION UPDATE FORM
This form shall be used to add or update a pro se litigant’s primary e-mail address for electronic service. In addition
to submitting this form, if the pro se litigant has any pending cases, the pro se litigant shall file and serve a copy of
a notice of change of address, upon all opposing parties, Local Rule 83-2.7.
