SUPERIOR COURT, COUNTY OF KERN IN THE MATTER OF THE DISPUTED CLAIM OF CASE NUMBER ORDER TO DEPOSIT MONEY A Minor The application of Trustee for authority to deposit the funds of said minor in the initial amount of $ in (Name and address of depository) is hereby approved. It is ordered that all money in the hands of said person which belongs to said minor, be forthwith deposited by said applicant and attorney in said depository in federally insured blocked account(s) by said applicant as trustee for said minor and that the blocked account(s) show the name of said minor. That all said money hereafter received, including interest, be forthwith deposited in said account(s). That no withdrawals of principal or interest be made from said account(s) without the written order under this case name and number therefor signed by a judge and bearing the seal of said court.
