INSTRUCTIONS: Submit this form along with a copy of a valid photo ID (i.e. Driver’s License, Identification
Card, School ID, birth certificate, passport) and any additional documents received from DMV to the following
court location:

Upon receipt of this form, the Court will send you, via U.S. mail, a letter indicating any outstanding citations and
the amount due for each citation. Payments will be accepted as follows:
Walk-in Payment: Payments may be made at any Informal Juvenile & Traffic Court branch. Payments will not
be accepted without the letter you have received from the court. If the letter is lost, you must appear in person
at the Central District location to submit payment.
Mail-in Payment: Send check or money order to the IJTC Central District only. DO NOT SEND CASH. Make
the check or money order payable to “LOS ANGELES SUPERIOR COURT”, include the citation number(s) on
the check or money order and include a copy of the letter received.
*General information about outstanding citations issued prior to October 22, 2007 can be
obtained at any IJTC branch location. Citations issued October 22, 2007 or after must
be handled in the branch location designated on the citation.
If you deny any of the unadjudicated charges on citations issued prior to October 22, 2007 you
must appear in person at the Central District, Monday through Friday at 1:30 p.m., to request a
date for a contested hearing. Hearing dates will be given in person only.

Frequently Asked Questions
Q. Can I request the information telephonically?
A. No, pursuant to WIC 827, juvenile cases are considered confidential and information regarding any
juvenile records will not be released telephonically.
Q. What do I do if I dispute the citation?
A. You must appear in person at the Informal Juvenile & Traffic Central District to request a future
hearing date. IJTC Central District is located at 1945 S. Hill Street, Room 808, Los Angeles, Ca
90007. The office hours are Monday through Friday from 8:30 a.m. to 4:30 p.m. Payments are taken
until 3:30 p.m. daily.
Q. What if I have an outstanding citation issued on or after October 22, 2007?
A. Any outstanding citations issued on or after October 22, 2007 must be handled at the court
location at which you were ordered to appear on the original citation. (For example: If you were
originally cited to the Pomona Branch Court on February 2, 2008, you must appear in the Pomona
Branch Court to resolve the matter).
Q. How long will it take to receive information back from the Court once I send in my request
form?
A. Upon receiving the request form, the clerk’s office will obtain your information for the citation(s),
which is on Microfilm. A bench officer will review the citation(s) and determine the appropriate fine,
and a letter is then mailed with the appropriate information. This process can take several weeks.
Q. Can I send a third party to pay my citation(s) once I receive the form back from the Court?
A. Yes, once we receive the information from you (photocopy of valid id, completed form with
signature), a third party may pay your citation as long as they are in possession of the letter you
received from the court. Payments can also be mailed to the address indicated on the form.
Payments sent by mail must be made by check or money order only and must include the form.
Checks and money orders must be made payable to Los Angeles Superior Court and should include
the citation number(s) on it.
Q. What if I lost the letter sent to me by IJTC?
A. If the letter is lost, you must appear at the Central District branch only to pay the citation.
