The spouse and the relatives within the second degree are entitled to notice. Second degree includes parents,
grandparents, children, adopted children, grandchildren, brothers and sisters, half brothers and half sisters.

Petition alleges that none of such relatives exist except as set forth below.

If spouse or a relative is deceased since last notice was given, please so indicate, giving date.
4.
If person is housed in other than your home, the director of that home must be notified when a
petition for appointment or petition to reestablish conservatorship is filed.
5.
Conservator must notify the department of Mental Health when a Petition to Reestablish
Conservatorship is filed.
