1. I, the undersigned proposed adoptive parent, having petitioned the above entitled Court for the approval of the adoption of
the proposed adoptee, do hereby agree to assume toward the proposed adoptee the legal relationship of parent and child
and to have all the rights and be subject to all of the duties and responsibilities of the relationship, including the right of
inheritance.
