A “Confidential Guardianship Status Report” has been enclosed with this Notice, and you are required to complete it, and return it the Probate Court at the address indicated in the heading of this Notice. You have also been scheduled for a Review Hearing on: Dept.: at the address noted above. at am/pm in YOU MUST RETURN THE “CONFIDENTIAL GUARDIANSHIP STATUS REPORT” ON OR BEFORE . IF YOU DO NOT RETURN THIS STATUS REPORT, YOU MAY BE SUBJECT TO FURTHER ORDER OF THE COURT WHICH INCLUDES REMOVAL AS THE GUARDIAN AND SANCTIONS (up to $1,500 per CCP 177.5). WILLFUL SUBMISSION OF FALSE INFORMATION IS GROUNDS FOR REMOVAL AND IS A MISDEMEANOR. Probate Code section 1513.2 (a). YOU MUST APPEAR AT THE TIME OF YOUR REVIEW HEARING. FAILURE TO APPEAR AT THE REVIEW HEARING IS GROUNDS FOR REMOVAL AND OR SANCTIONS (up to $1,500 per CCP 177.5) BY THE COURT. At the time of the review hearing, the Court will review your ”Confidential Guardianship Status Report” and may ask you some questions. The Court will then make further orders which may include approving your report and extending your guardianship for another year at which time another Review Hearing and “Confidential Guardianship Status Report” will be required. After your hearing, you will need to obtain new “Letters of Guardianship”. Staff onsite will direct you how to obtain these forms. To obtain information regarding the status of your Review Hearing or a recently submitted “Confidential Guardianship Status Report”, you may call the Probate Examiner between 1:30 – 2:30 pm Monday –Friday at the numbers listed above.
