MEDIATION OUTCOME QUESTIONNAIRE
This questionnaire is part of the evaluation of the San Francisco Superior Court's voluntary civil
mediation program. Your cooperation in the evaluation process is greatly appreciated. We encourage
you to be candid, particularly in offering suggestions for improvement of the program. For this reason all
responses will be treated confidentially. Your response will be combined with the responses of others
and information from the case file itself and will be reported only in group or aggregated form.
