Form CONV LLC-GS has been created for ease in filing, however, you can compose your own document, provided it meets statutory requirements. This form only can be used for a California limited liability company converting to a California stock corporation by a majority vote of the members. To file this Form CONV LLC-GS, the California limited liability company must be active on the records of the California Secretary of State.
