STATE OF CALIFORNIA DEPARTMENT OF INDUSTRIAL RELATIONS WORKERS' COMPENSATION APPEALS BOARD ARBITRATION SUBMITTAL ORDERS CASE NAME I.D. OR CASE NO. ORDER FOR SUBMITTAL TO PANEL ARBITRATOR An Arbitration Submittal Form having been properly filed and selection of an arbitrator pursuant to Labor Code Section 5271(b) having been completed. IT IS ORDERED that the issues checked on the attached Arbitration Submittal Form filed on be arbitrated before (Name of arbitrator) pursuant to Labor Code Sections 5272, 5273, 5276 and 5277 and Workers' Compensation Appeals Board Rules 10995, et seq. The arbitrator's award shall be served on all parties and filed with the Appeals Board within 30 days from the submission by the arbitrator of the case for decision. The arbitrator's award/decision shall comply with Labor Code Section 5313. PRESIDING WORKERS' COMPENSATION JUDGE DATE ORDER FOR SUBMITTAL TO AGREED ARBITRATOR The attached arbitration submittal is hereby approved.
