Use this form to resign as a Registered Agent for multiple entities. There is no fee for this form.
This form is to be used only for Registered Agents that serve multiple entities in the District of Columbia.
Under DCOC Titles 29 and 41, the RA must mail or deliver directly to the entities a copy of this form and file 2 copies with the Corporations
Division, 10 days after that. The RA's appointment will end 30 days after the Corporations Division's receipt of the notice or when the entity
appoints a successor agent, whichever occurs first. The Corporations Division will return the file-stamped copy of this notice to the resigning
RA. Indicate the name and file number for each resigned entity.
