IN THE CIRCUIT COURT FOR FLORIDA IN RE: GUARDIANSHIP OF COUNTY, PROBATE DIVISION File No. Division ORDER DISAPPROVING INITIAL GUARDIANSHIP PLAN OF GUARDIAN OF PERSON AND COMPELLING COMPLIANCE The Court having reviewed the Initial Guardianship Plan filed by as guardian of the person of (the Ward), and having reviewed the report of the Clerk of the Court, finds that the Initial Guardianship Plan is not complete as indicated below: The Guardianship Plan fails to provide, or fails to provide adequately, the following information as required by Florida Statutes Section 744.363. () () () () Provision of medical, mental or personal care service for the welfare of the Ward. Provision of social and personal services for the welfare of the Ward. The place and kind of residential setting best suited for the needs of the Ward. The application of health and accident insurance and any other private or governmental benefits to which the Ward may be entitled to meet any part of the costs of medical, mental health, or related services provided to the Ward. Physical or mental examinations necessary to determine the Ward's medical and mental health treatment needs. An attestation that the guardian has consulted with the Ward and, to the extent reasonable, has honored the Ward's wishes consistent with the rights retained by the Ward under the plan.
