Box 880 Lawrenceville, GA 30046 Step Two: Certificate of Appointment • After the Clerks Office has received your application, your Certificate of Appointment will be mailed to you. Please sign all three copies at the vertical line on the left side of the form indicating “Notary’s Signature.” Step Three: Final Phase • • • Mail the signed Certificates back to the Clerks Office in the envelope provided. (Do not have these certificates notarized.) Checks and money orders in the amount of $30.00 should be made payable to: Clerk Gwinnett Superior Court You will receive your official copies after your application has been granted and payment has been received. Pursuant to O.C.G.A. 45-17-8.1(a), a notary public shall sign in ink only and exactly the name indicated on the notary’s commission. Pursuant to O.C.G.A. 45-16-6 (a)(1), Notaries seal can be either a rubber stamp seal or an embossed seal and must show the Notary name as shown on commission and the words “Notary Public”, the name of the state and the county of appointment.
