Make one copy for your Employer. 11. Give the original and all copies to the court at the hearing. The court will fill in the rest of the form once the hearing has been held. You will need to provide self-addressed stamped envelopes for yourself, the other party and your Employer. The Clerk of the Court will mail the copies after the order has been signed by the judge.
