INSTRUCTIONS FOR ALL ALCOHOL BEVERAGE LICENSES
Forms must be legible (Print or Type)
All blanks must be completed. Follow all instructions printed on each form. Use N/A if the
question does not apply. An incomplete application will be denied.
Forms must be signed and notarized.
Fingerprint cards must be submitted for each new individual, partner, corporation officer (board of
directors), LLC member. The fee for each fingerprint card is $34. This must be paid when the
cards are submitted. (One check for all fingerprints submitted, separate from the application fee).
DO NOT FOLD, TEAR OR STAPLE THE CARDS.
Express mail envelopes for return service will be used only when provided by the applicant who
must be listed as both the sender and receiver. The postage must be pre-paid.
Fees must accompany applications. Make checks payable to “State of Idaho.” Do not mail cash.
Completing the application
1.Complete New, transfer or change
2.Complete license type & fees
Fees: Complete as indicated on cover sheet. If transferring the license, a $20 fee for each
type (keg, beer, wine) is required. The fee for transferring a liquor license through a lease is
half of annual fee; through a purchase is10% of purchase price of license or goodwill,
whichever is greater.
A Restaurant Is: A full service restaurant with kitchen facilities, a printed menu (not Xerox Copy)
regularly served meals.
3. Complete applicant information. The applicant is the Individual(s), partnership, corporation,
LLC, or association that will be conducting the business. All information needs to apply to the
applicant.
Complete the entire application. Additional items needed:
Fingerprint cards and fees for each person listed on the application
Current building lease or proof of ownership (warranty deed) in the name of the applicant
Printed menu (for all restaurant applicants)
Current zoning approvals from the city or county with jurisdiction
Articles of Incorporation or Organization (Corporations/LLC’s)
Bylaws (Liquor license or wholesale Corporations)
Building Approval (Buildings will not be inspected until they are completed)
Partnership Agreement: (copy) which has been filed with the Secretary of States Office.
Fees. Make checks payable to “State of Idaho”
A liquor license lease or bill of sale must be included with all liquor license applications.
