CONSENT TO TRIAL BY UNITED STATES MAGISTRATE JUDGE
In accordance with the provisions of 28 U.S.C. 636(c) and Fed.R.Civ.P.73, the parties, being
fully aware of the right to trial before a United States District Judge, do hereby consent to allow a
United States Magistrate Judge of the Southern District of Illinois to conduct all proceedings in this
matter, including but not limited to the trial of this case, and to order and direct the entry of
judgment herein.
The parties further consent that upon entry of judgment herein, any aggrieved party may
appeal directly to the Seventh Circuit Court of Appeals in the same manner as an appeal from any
other judgment of a District Court.
IT IS FURTHER AGREED that nothing contained herein shall be deemed to preclude any
duly appointed and qualified District Judge of this Court from conducting any and all proceedings,
including trial of the above-designated case.
