Pursuant to the Juvenile Court Act, 705 ILCS 405/2-25, respondent(s) shall observe the following conditions (check only those that apply): a. ensure appropriate supervision and provide all care (including appropriate medical care) for the minor(s) during visits; b. cooperate and comply with all reasonable requests and referrals made by DCFS and its agents or assigns, and sign releases of information for all referrals made; c. notify DCFS and its agents or assigns prior to any change of address; d. notify DCFS and its agents or assigns prior to any change in living situation; including but not limited to the addition of other persons into the household; e. not use, or allow anyone else to use, any corporal punishment on the minor(s) (no spanking or hitting with any objects, including but not limited to belts, cords, sticks, fists or hands); f. refrain from using or possessing any illegal substances or paraphernalia; g. refrain from using alcohol during visits; h. provide samples for random drug screens as requested; i. notify DCFS or its assigns or agents, within 24 hours of any injury to the minor(s) (including any bump, bruise, cut, scratch or fall) which would require professional medical treatment; j. arrive promptly to visits and provide a minimum of 24 hours notice if unable to attend a visit; k. ensure that during respondent's visits the minor(s) has (have) no contact with ___________________ ____________________________________________________________________________________; l. ensure that during respondent's visits the minor's (s') contact with ____________________________ ___________________ is limited to _____________________________________________________; m. other: _______________________________________________________________________________ ____________________________________________________________________________________ 4.
