The undersigned hereby certifies under penalty of perjury as provided by the Illinois Code of Civil
Procedure, that I am a party to this action and I hereby certify and stipulate as follows:
1.
My spouse and I have lived separate and apart for a continuous period of not less than six (6)
months.
2.
Irreconcilable differences have caused the irretrievable breakdown of this marriage.
3.
Efforts at reconciliation have failed.
4.
Future attempts at reconciliation would be impracticable and not in the best interest of the family.
5.
We hereby waive and relinquish the two (2) year waiting requirement of living separate and apart.
6.
This Certificate and Stipulation is being made knowingly and without coercion. We wish to be
bound by this CERTIFICATE AND STIPULATION.
