ADJUDICATORY ORDER AND DISPOSITIONAL ORDER
THIS MATTER comes before the Court for hearing on the date noted above with the
parties indicated being present. The parties have been advised of the nature of the proceedings as
well as their rights and the dispositional alternatives available to the Court. The minor admits the
allegations of Count I ____________________ of the Petition for Adjudication filed
_______________. The Court makes the following FINDINGS:

The Court has jurisdiction of the subject matter.
The Court has jurisdiction of the parties.
The admission by the minor is knowingly and voluntarily made.
The minor has signed an Admission form.
There is a factual basis for the admission by the minor.
The parties have agreed to a dispositional recommendation with regard to this
matter.
THEREFORE, it is the ORDER of this Court that the request of the parties for an
immediate dis-positional hearing is GRANTED.

THIS MATTER then proceeds to dispositional hearing. Both parties waive the
preparation of a social investigation. The agreement of the parties is heard. The Court makes the
following FINDINGS:

The agreement of the parties is in the best interest of the minor.
The agreement of the parties should be affirmed and incorporated in the
Dispositional Order of this Court.
THEREFORE, it is the ORDER of this Court that:
1.
The minor is adjudicated to be a delinquent minor.
2.
The minor is made a ward of this Court.
3.
The minor is placed on probation pursuant to 705 ILCS 405/5-23 for twelve (12) months
from the date of this Order.
4.
This probation is conditioned upon the following terms and conditions:
i.
The minor shall not violate any Federal or state law or municipal ordinance.
ii.
The minor shall not leave the State of Illinois without the permission of the Court.
iii.
The minor shall report to the __________________________________________ or
any other probation officer providing courtesy supervision of the minor.
iv.
The minor shall truthfully answer any question asked by any probation officer.
v.
The minor shall not possess a firearm or other dangerous weapon. (Hunting is not
permitted during the term of this Order.)
vi.
The probation officer is authorized to visit at any time and in any location.
vii.
The probation officer may search the minor and the minor's personal possessions
at any time.
viii.
The probation officer may require the minor to undergo drug or alcohol tests at any
time.
ix.
The minor shall observe a curfew which shall be established by the Probation
Officer.
x.
The minor shall attend school at all times that school is in session. Any absence,
tardy or truancy not caused by verifiable illness is a violation of this Order.
Suspension or expulsion is also a violation of this Order.
xi.
The minor shall cooperate with and participate in any and all social services which
may be required by the probation officer.

xii.
The minor shall comply with all rules, regulations and orders of the PROBATION
OFFICE.
THIS ORDER MAY BE MODIFIED BY THE COURT.
