DISPOSITIONAL ORDER
THIS MATTER comes before the Court for dispositional hearing on the date noted
above with the parties indicated being present. An Adjudicatory Order was previously entered in
this matter on ________________________. A Social Investigation has been prepared by the
PROBATION OFFICE, and is made a part of the record of this case. No evidence is present by any
of the parties. Recommendations of the parties are heard. The Court makes the following
FINDINGS:

The minor has serious problems which need to be addressed.
Due to the age of the minor, if treatment does not occur at this time, there may
never be another opportunity for treatment to be effective.
There has been a failure on the part of the mother to invest herself in the well-being
of the minor by determining the nature and extent of the problems of the minor and
seeking solutions to those problems.
The mother must understand that the responsibility for the minor rests with her
and she must actively participate in the treatment program.

THEREFORE, it is the ORDER of this Court that:
1.
The minor is adjudicated a delinquent minor.
2.
The minor is made a ward of this Court.
3.
The minor is placed on probation pursuant to 705 ILCS 405/5-23 for eighteen (18)
months from the date of this Order.

This probation is conditioned upon the following terms and conditions:

The minor shall not violate any Federal or state law or municipal ordinance.
The minor shall not leave the State of Illinois without the permission of the Court.
The minor shall report to the Juvenile Probation Officer.
The minor shall truthfully answer any question asked by any probation officer.
The minor shall not possess any firearm or other dangerous weapon. (Hunting is
not permitted during the term of this Order.)
The probation officer is authorized to visit at any time and in any location.
The probation officer may search the minor and the minor's personal possessions
at any time.
The probation officer may require the minor to undergo drug or alcohol tests at
any time.
The minor shall observe a curfew which shall be established by the Juvenile
Probation Officer.
The minor shall attend school at all times that school is in session. Any absence,
tardiness or truancy not caused by verifiable illness is a violation of this Order.
Suspension or expulsion is a violation of this Order.
The minor shall cooperate with and participate in any and all social services which
may be required by the probation officer. Specifically, the minor shall be evaluated by and make satisfactory progress in counseling with the YOUTH SERVICE
PROGRAM.
The minor shall comply with all rules, regulations and orders of the PROBATION
OFFICE.

Arrangements shall be made to have an IEP evaluation completed concerning the
educational placement of the minor within the next thirty (30) days.
6.
The mother of the minor is placed under the supervision of the PROBATION OFFICE
pursuant to 705 ILCS 405/5-26. The terms and conditions of protective supervision
include:

The mother is to actively perform all activities with respect to this minor commonly associated with parenting including, but not limited to, providing for the
care, guidance, education, discipline and support of the minor.
The mother shall engage in counseling to educate herself as to the problems of the
minor and how to deal with those problems on a personal basis.
She shall make all appointments for herself and make sure that the minor makes
all of his appointments.
She shall cooperate with all social service providers.
The mother will arrange for a psychiatric evaluation of the minor and follow
through with the recommendations coming from that evaluation.
The mother will make sure that the minor is taking his medication.
The mother shall actively participate in the educational process of the minor
including attendance at parent/teacher conferences and placement staffings;
checking on the attendance of the minor; being informed as to the progress or
problems of the minor; and, making sure that the work of the minor is completed
in a timely, appropriate fashion.
The mother is primarily responsible for initiating and following through with
respect to these matters but should seek the assistance of the probation officer if
problems develop. The probation officer is to assist in a supportive role and to
deal with bureaucratic problems rather than to be the primary motivator with
respect to services for the mother or the minor.

I have read and understand the terms and conditions of this Order. I further understand that any
violation may result in a detention hearing for the minor and a contempt hearing for the mother
as well as possible detention until this case is resolved or contempt sanctions . I also state that I
