THIS MATTER comes before the Court for hearing on the date noted above with the
parties indicated being present. The case is called for dispositional hearing. A social investigation has been prepared by the Juvenile Division of the PROBATION OFFICE and is made a part of
the record. None of the parties present any evidence. Recommendations of the parties are heard.
The minor makes a statement on his own behalf. The Court makes the following FINDINGS:

allegations of all pending petitions were dismissed on motion of the Petitioner. An
Adjudicatory Order was entered on _____________ which, among other things,

The minor should be adjudicated to be a delinquent minor.
The minor should be made a ward of the Court and the minor should be committed
to the ILLINOIS DEPARTMENT OF CORRECTIONS, JUVENILE DIVISION for the following
reasons:
i.
The record in this case and the Social Investigation clearly show that the
minor has not and will not cooperate with the Court or anyone else
attempting to provide social services to him.
ii.
The minor poses a significant threat to the persons in this community.
iii.
Considering the risk which the minor poses to the community, a disposition other than commitment to the ILLINOIS DEPARTMENT OF CORRECTIONS,
JUVENILE DIVISION would not only deprecate the seriousness of the actions of
the minor but indicate to the minor that there are virtually no consequences
for the actions in which he voluntarily engages.
THEREFORE, it is the ORDER of this Court that:
1.
The minor is adjudicated to be a delinquent minor.
2.
The minor is made a ward of the Court.
3.
The minor is COMMITTED to the ILLINOIS DEPARTMENT OF CORRECTIONS, Juvenile
Division.
4.
Custody of the minor is given to the Assistant Director, ILLINOIS DEPARTMENT OF
CORRECTIONS, Juvenile Division.

5.
The minor is placed in the custody of the SHERIFF OF ____________ COUNTY, ILLINOIS
for transport to the ILLINOIS DEPARTMENT OF CORRECTIONS, Juvenile Division.
6.
Pending transport to the ILLINOIS DEPARTMENT OF CORRECTIONS, Juvenile Division
the minor shall be held at the ____________________ Juvenile Detention Center. If
space is not available at the _____________________ Juvenile Detention Center the
minor should be detained at another authorized juvenile detention facility.
