ORDER CONTINUING THE CASE
UNDER THE SUPERVISION OF THE COURT
THIS MATTER comes before the Court for hearing on the date noted above with the
parties indicated being present. The parties have been advised of the nature of the proceedings as
well as their rights and the dispositional alternatives available to the Court. The minor admits the
allegations of the Petition for Adjudication filed ________________. The Court makes the
following FINDINGS:

Continuing this matter under the supervision of the Court pursuant to 705 ILCS
405/5-19 is in the best interest of the minor.

THEREFORE, it is the ORDER of this Court that:
1.
No finding or adjudication is entered of record regarding the allegation of delinquency.
2.
The case is continued under the supervision of the Court pursuant to 705 ILCS 405/5-19
for twelve (12) months from the date of this Order.
3.
This continuance is conditioned upon the following terms and conditions:

The minor shall not violate any Federal or state law or municipal ordinance.
The minor shall report to the Juvenile Probation Officer.
The minor shall truthfully answer any question asked by any probation officer.
The minor shall not possess a firearm or other dangerous weapon. (Hunting is not
permitted during the term of this Order.)
The probation officer is authorized to visit at any time and in any location.
The probation officer may search the minor and the minor's personal property at
any time.
The minor shall observe a curfew that will be provided to the minor in writing by
the probation officer.
The minor shall attend school at all times that school is in session. Any absence,
tardiness or truancy not caused by a verifiable illness is a violation of this Order.
Suspension or expulsion is a violation of this Order.
The minor and the parents, guardians, or custodians of the minor shall cooperate
with and participate in any social services that may be required by the probation
officer. Specifically, the minor shall be evaluated by and make satisfactory
progress in counseling with the Youth Service Program.
The minor is to comply with all rules, regulations and orders of the PROBATION
OFFICE.
The parents, guardians or custodians of the minor are responsible for making sure
that the minor is in compliance with the terms and conditions of this Order and are
required to execute any releases or permission forms to fulfill the requirements of
this Order.
The minor shall draft a letter of apology to ______________. The letter is to be approved
and sent by the Probation Officer. The minor is to continue revising the letter until
Probation is satisfied with the content of the letter.
THIS ORDER MAY BE MODIFIED BY THE COURT.
