ORDER SUBSEQUENT TO PERMANENCY REVIEW HEARING
THIS MATTER comes before the Court for hearing on the date noted above with the
parties indicated being present. The hearing is held pursuant to Title IV of the Social Security
Act Section 427 as amended by the Federal Adoption Assistance and Child Welfare Act of
1980 and for permanency review pursuant to 705 ILCS 405/2-28. The Court reviews the client
service plan; the appropriateness of the permanency goal; the appropriateness of the services
contained in the plan and whether those services have been provided; whether reasonable efforts
have been made by all parties to the service plan to achieve the goal; and, whether the goal has
been achieved. The Court hears evidence and reviews the reports which have been submitted.
The Court makes the following FINDINGS:

The Court has jurisdiction of the subject matter and of the parties.
The goal of Returning Home Within One (1) Year is found to be appropriate.
The services have been designed to achieve the goal.
The goal cannot be achieved immediately due to the necessity for continued
treatment of the minor in a residential treatment facility.
The minor has done very well in treatment and a plan for transition from the
treatment facility to home is to be provided at the next permanency review hearing.
The services required by the Court and the service plan have been provided.

THEREFORE, it is the ORDER of this Court that:
1.
The Guardianship Administrator of the ILLINOIS DEPARTMENT OF CHILDREN AND
FAMILY SERVICES continue to act as the guardian of the minor with the power to consent
to medical, surgical, dental, and psychological services for the minor in consultation with
the mother.
2.
The services contained in the plan continue to be provided.
3.
This matter is set for permanency review hearing on ___________________ at
_______________________________.
