I understand that by acknowledging paternity, I have certain rights and duties in connection with this
matter. I am voluntarily giving up these rights and assuming the obligations of parenthood, which may
include being ordered to contribute to the support and welfare of the child until he or she is 18 years of
age, or as otherwise provided by the court, and which may also include being ordered to pay for
pregnancy and delivery costs of the mother.
I further understand that if I choose not to sign this acknowledgment I have the following rights:
1. If I am unable to afford a lawyer, the court may appoint me one free of charge and also provide
me free transcripts of the proceedings.
2. I have the right to a trial before a judge.
3. I have the right to have the court order me, the mother and the child to submit to blood tests to
determine if I am the father. The court could order that I pay all or some of the costs of the blood
tests, depending in part on whether I can afford to pay.
