MOTION TO TERMINATE SUPPORT
1. I am the named obligor under a child support order in this case.

The youngest child under that order has now reached the age of 18 and has graduated from
high school,
or
The youngest child under that order will reach the age of 18 on
and has graduated from high school.
3. My child support obligation is current and I owe no support arrearage.
4.
I have given timely notice to the other party in the manner required by Supreme Court Rule 11,
or
I have obtained the written agreement and consent of the other party to the entry of this order.
Therefore, I request that an order be entered terminating the Order of Support previously entered
in this case.
Under penalties as provided by law pursuant to Section 1-109 of the Code of Civil Procedure, I
certify that the statements set forth in this instrument are true and correct.
