PLEASE TAKE NOTICE pursuant to 750 ILCS 60/214(14.5) that an Order of
Protection, a certified copy of which is attached hereto, has been entered this date
in the Circuit Court of Lake County, Illinois.
Pursuant to the terms of the Order of Protection, Respondent has been
ordered to turn over certain firearms for safekeeping to the above law enforcement
agency. Please refer to Part D of the Order of Protection for the list of weapons and
the period of safekeeping.
