PLEASE TAKE NOTICE pursuant to 750 ILCS 60/222 (e) that an Order of
Protection, a certified copy of which is attached hereto, has been entered this date
in the Circuit Court of Lake County, Illinois.
Pursuant to 750 ILCS 60/222 (f) access to a protected child’s school/day care
facility records or release of information in those records by the above school/day
care facility, or its employees, to the respondent is prohibited. The school/
school/day care facility shall file a copy of the order of protection in the records of
each protected child.
