This cause having come before the Court upon the Verified Petition for Name Change filed herein, the

THE COURT FINDS:
That all material facts alleged in said Petition have been proved sufficiently; that said Minor is now and
has been a resident of the State of Illinois for at least six months prior to the time of filing the Petition herein;
that the conditions mentioned and specified in 735 ILCS 5/21-101 et seq. have been complied with; that this
