DECREE (MINOR)
This cause having come to be heard upon the Verified Petition filed herein, and the Court having
considered said Petition; and being fully advised in the premises, doth find:
That all material facts alleged in said Petition have been proved sufficiently; that said Petitioner
is now and has been a resident of the State of Illinois for six months or more prior to the time of
filing the Petition herein; that the conditions mentioned and specified in 735 ILCS 5/21 have
been complied with; that this Court has jurisdiction of the Petitioner, has considered the relevant
factors as specified by statute, and finds that the prayer in the Petition should be granted and that
said change of name is necessary to serve the best interest of the minor Petitioner.
IT IS THEREFORE ORDERED, ADJUDGED AND DECREED that said Petitioner’s name be
changed from ______________________________ to ________________________________
by which last mentioned name Petitioner shall be hereafter known and called.
