The personal identifying information regarding the Petitioner Respondent Other Party in subsections 6, 7, 9(b) and 18 is not required to be provided because there is a history of domestic violence or abuse. 5. The Court finds that the Petitioner Respondent Other Party is not required to provide the personal identifying information stated in subsections 6, 7, 9(b) and 18 because disclosure is not in the best interests of the child(ren) or party. 6. Parental Conduct: a) Day-to-day decisions such as minor training or correction, minor medical and dental care, chores, allowances, clothing, hygiene, etc. will be made by the party who has the child(ren) at the time such decisions are necessary. b) The Petitioner Respondent Both Parties shall provide to the other party at least sixty (60) days prior written notice of the intention to change his/her residence. At minimum the party changing his/her residence shall provide the intended date of the change of residence and the address of the new residence. If the sixty (60) day notice is impracticable, written notice shall be given at the earliest practical date. c) Both parties shall provide the names, addresses, and telephone numbers of all medical, dental and mental health care providers. Either party may authorize emergency care, but if possible, both parties shall contact the other party first. d) Pursuant to state law, both parties have access to the records of the child(ren) including school, medical, dental, and mental health records, pursuant to 750 ILCS 5/602.11. [For good cause shown, this Court orders that the Petitioner Respondent Other Party shall not have access to the records of the child(ren) set forth under this section]. e) Travel and Vacation Plans: If either party requires out of state or any type of overnight travel with the child(ren), each party shall inform the other party of such travel and vacation plans, including notice and contact information. 10.
