GSD-14 is a required when the air permit application submitted to IDEM, OAQ is subject to the notification requirements outlined in Title 13, Article 15, Chapter 8 of the Indiana Code (IC 13-15-8). For the purposes of this form, the term “source” refers to the plant as a whole and NOT to the individual emissions units.
