The purpose of GSD-15 (State Form 51608) is to identify local government officials that are to be notified that an air
permit application has been submitted. GSD-15 is a required form for all air permit applications submitted to IDEM, OAQ.
For the purposes of this form, the term “source” refers to the plant as a whole and NOT to the individual emissions units.

This table identifies local government officials that should be notified, pursuant to Title 13, Article 13, Chapter 3, Section 1
of the Indiana Code (IC 13-13-3-1), that an air permit application has been submitted. If you need additional space, you
may make copies of this form.

Provide the names of local government officials that should be notified pursuant to IC 13-13-3-1 that an air permit
application for this source has been submitted.
2. Date Notified:
If applicable, provide the date on which you notified the local government official(s) of your intent to construct a new
source of air pollutant emissions.
3. Title:
Provide the title of each local government official listed on this form.
4. Address:
Provide the mailing address for each local government official listed on this form.
5. Electronic Mail:
If available, provide the electronic mail (email) address for each local government official listed on this form.
6. Telephone Number:
Provide the telephone number for each local government official listed on this form.
7. Method of Notification:
Indicate the preferred method of notification for each local government official listed on this form, if known.

confidentiality. Claim of confidentiality must be made at the time the information is submitted to IDEM, and must follow
