The purpose of GSD-11 (State Form 51601) is to identify and summarize any Alternative Operating Scenario (AOS) that
you are submitting with your air permit application. You should complete a copy of this form for each AOS that you
include with your air permit application. If you do not provide the enough information to adequately describe each AOS,
the application process may be stopped. GSD-11 is optional for all air permit applications submitted to IDEM, OAQ. For
the purposes of this form, the term “source” refers to the plant as a whole and NOT to the individual emissions units.

Alternative Operating Scenario Summary
This table identifies and summarizes any alternative operating scenario (AOS) that you are submitting with your air permit
application. An AOS gives the Permittee the option to operate the process according to one of two or more pre-set
operating scenarios as long as records are kept documenting which scenario was followed at any point in time. Some
examples of AOS may include: use of an alternate fuel, use of a different raw material, production of a different final
product; and/or using a different method of compliance. If you are not sure if a process would be considered a separate
process or an AOS, contact the PROD for additional guidance.
1. Process:
Provide a brief description of the process for which you are providing additional information.
2. Unit ID:
Provide the identification number for each emissions unit for which you are providing additional information. The
identification numbers listed on this form should correspond to the emissions unit identified on the Plant Layout and
Process Flow diagrams.
3. Stack / Vent ID:
Provide the identification number for each stack or vent to which the AOS applies. The identification numbers listed
on this form should correspond to the stacks and vents identified on the Plant Layout and Process Flow diagrams.
4. AOS ID:
You must assign a unique identification number to each AOS. The identification number listed on this form should
correspond to each AOS identified on the Plant Layout and/or Process Flow diagrams.
5. Application Form Reference:
Identify each IDEM application form as well as the specific item number to which the AOS applies.
6. Explanation:
Provide a brief explanation of why the operation should be considered an AOS rather than a new process.
7. AOS Description:
Provide a brief description of the AOS in terms of process information, any control equipment to be utilized, methods
to be used to demonstrate compliance, and the resulting emissions under AOS conditions.
8. Additional Calculations / Diagrams:
Use the space provided to include additional calculations and/or diagrams, if applicable.

All information submitted to IDEM will be made available to the public unless it is submitted under a claim of
confidentiality. Claim of confidentiality must be made at the time the information is submitted to IDEM, and must follow
the requirements set out in 326 IAC 17.1-4-1. Failure to follow these requirements exactly will result in your confidential
information becoming a public record, available for public inspection. To ensure that your information remains
confidential, refer to the IDEM, OAQ information regarding submittal of confidential business information. For more
