CERTIFICATION OF AIRCRAFT REGISTRATION NAME CHANGE

This form is to be used to change the name on aircraft registration
and to determine state tax liabilities or exemptions
INSTRUCTIONS:
1. IN ALL SITUATIONS, YOU MUST COMPLETE THE AFFIDAVIT BELOW.
2. If you are a CORPORATION and you are:
a. FORMING A CORPORATION, attach to this form a certified copy of your corporate minutes (signed by a corporate officer and
notarized) proving the aircraft in question to be part of the original contributed capital.
b. CHANGING YOUR CORPORATE NAME, attach to this form a certified copy of your corporate minutes (signed by a corporate
officer and notarized) detailing the specifics of the name change.
c. MERGING two or more Corporations, attach to this form a certified copy of your corporate minutes (signed by a corporate officer
and notarized) detailing the Board of Director’s vote to accept the merger.
d. WHOLLY-OWNED SUBSIDIARY formerly titled by a parent corporation, attach to this form a copy of your corporate minutes
(signed by a corporate officer and notarized) that clearly indicates the ownership of the subsidiary corporation.
NOTE: All cases of CORPORATE DISSOLUTION (wherby a corporation’s aircraft registration has been assigned to a private
individual) are considered to be a change of ownership, Indiana Sales (use) tax payment is due upon the transaction. Furthermore,
the individual may neither use the corporation’s registration certificate nor benefit from any previous excise tax payment by the
corporation.
3. BUSINESS OTHER THAN CORPORATION need only submit this form, with the affidavit below bearing the signature of your president,
one of the partners, or the principal owner.
4. When completed, send this form and all applicable documents to: INDIANA DEPARTMENT OF REVENUE, SPECIAL TAX –
AERONAUTICS, P.O. BOX 644 ,INDIANAPOLIS, INDIANA 46206-0644 PHONE: (317) 615-2544
After receiving this form, the Special Tax – Aeronautics will notify you as to the extent of any tax liability or exemption. Please do not hesitate to
