You are required to file this report even if you have no sales. COMPLETING THE WINERY SHIPPING PERMIT ANNUAL REPORT OF SALES: 1. Complete the year and enter your FEIN. Enter your Winery Shipping Permit number and check the appropriate box for your permit type. 2. Complete your Winery Shipping Permit name, demographic and contact information. 3. If you have no shipments to report this year, check the box “I have no shipments to Kansas consumers this year” and file your report. 4. To report shipments, complete information listed for each order as follows: A. Order Date. Enter the date the product was ordered by the customer. Report all orders received during the calendar year you are reporting for, even if they have not been shipped. B. Order Number. Enter the invoice number that identifies the order. C. Customer Name. Enter the name of the customer who placed the order. D. Shipment Date: Enter the date the order was shipped. E. Shipped To: • Distributor. (Large Direct Shipping Permitees only). Enter the license number of the licensed Kansas Distributor to whom the order was shipped for delivery to the licensed Retail Liquor Store designated by the customer. Enter the number of gallons shipped. • Light Wine. Enter the number of gallons shipped. H. Order Total. Enter the total as reported on the ABC-189 Kansas Winery Shipping Document. 5. Total Number of Gallons. Enter the total number of gallons of Fortified Wine and Light Wine Shipped. Report whole gallons only, rounding up to the nearest gallon. DEFINITIONS: Fortified Wine: Contains 14.1% ABV or more. Light Wine: Contains 14% ABV and less.
