The following Application is to be completed by attorneys requesting appointment to the CJA
list of individuals eligible to represent indigent defendants under the provisions of the Criminal Justice
Act. Attention of the applicant is called to the Criminal Justice Act of 1964, as amended. 18 U.S.C.
§3006A.
