ATTORNEY REQUEST FOR TWO MAILING ADDRESSES
Instructions: This form must be completed by an attorney with a valid Massachusetts BBO number and sent
directly to the Superior Court Administrative Office at the address below. The form should be submitted only
if the attorney wants to have his/her mail in some Superior Court cases sent to a “PRIMARY
ADDRESS”and mail in other cases within the Superior Court sent to a different address,“SECONDARY
ADDRESS”. The Administrative Office staff will send written notification to the attorney completing this form
when his/her multiple address records have been added. Thereafter it is the ATTORNEY’S
RESPONSIBILITY to properly notify the Clerk’s Office in each county in which you practice as to the
appropriate address to be used in each case for which you have filed an appearance.
