Request by Adult Adoptee for Identifying Information Release of Information Authorization Adult Adoptee Parent’s Consent/Denial to Release Information to Adult Adoptee Other: The appropriate fee. Check or Money Orders (Money Order for all Out of State requests) are made payable to: Oakland County Clerk. $10 – for each certified copy $20 - to send Clearance to Central Registry in Lansing $60 - non-identifying information request $20 - filing fee for the Petition for Confidential Intermediary $250 - the Confidential Intermediary search A copy of your driver’s license or picture identification. A copy of your non-identifying information already obtained. Proof of relationship to adoptee (cc of birth certificate to show common parent).
