A limited liability partnership whose statement of qualifiation has been revoked may apply to the Secretary of State for
reinstatement by filing a single annual renewal and paying a reinstatement fee of $160.00.
If changes to Lines 3 through 7 are necessary, a “Statement of Amendment or Cancellation” must be completed and
included along with the annual renewal form and an additional $135.00 filing fee.
