ORDER ADOPTING STANDARDIZED DIVORCE FORMS
WHEREAS,
the
Supreme
Court
Commission
on
Law
Libraries, together with the Supreme Court Pro Se Assistance Council and
the District Court Commission on Law Libraries, have petitioned this
court to adopt the following standardized forms that may be used in
divorce cases in the courts of the State of Nevada:

WHEREAS, this court has considered the petition and
proposed forms and determined that standardized forms for use in divorce
cases in the courts of the State of Nevada will facilitate access to the
courts by self-represented litigants, accordingly,
IT IS HEREBY ORDERED that
1. Effective immediately, the standardized forms developed by
the Commissions, copies of which are attached to this order as Exhibits A
through R, may be used in divorce cases in the courts of the State of
Nevada and, when those forms are used, the courts of the State of Nevada
shall accept them;
2.
The
Supreme
Court
and
District
Court
Library
Commissions shall work with the Administrative Office of the Courts and
the Family Law Section Executive Council to develop a training program
to educate law librarians, court personnel, and the general public on the
adoption and use of the standardized forms;
3.
The
Supreme
Court
and
District
Court
Library
Commissions shall also assist courts in developing standard instructions
for use by pro se litigants on administrative matters, such as filing
documents and scheduling hearings encompassing the practice in an
individual district; and

4. The Supreme Court Pro Se Assistance Council shall meet
on or about July 1, 2005, to review the standardized forms in light of the
experience with their use and in light of any changes in applicable law
approved during the 2005 Nevada legislative session and shall recommend
any necessary revisions to the forms to this court.
