DEBTOR’S REQUEST FOR DETERMINATION UNDER § 109(h)(4)
The debtor named above hereby certifies that the debtor is unable to obtain budget and
credit counseling because of :
G incapacity or disability as defined in 11 U.S.C. § 109(h)(4); or
G active military duty in a military combat zone.
Therefore, the debtor requests that the court make a determination that no budget and
credit counseling is required pursuant to 11 U.S.C. § 109(h)(4).
