of the state which had jurisdiction over the person named above, stating that on the date shown below, the person
named above was:
1. unconditionally discharged by that agency.
2. pardoned by the President of the United States or the Governor of the state in which the person named above was
convicted, and that his/her pardon was:
a. unconditional.
b. conditional and the person named above has satisfied the conditions of the pardon.
By virtue of Section 13-1 of the North Carolina General Statutes, the citizenship rights of the person named above have
