SECTION II: PARENTING SCHEDULE - IF APPLICABLE ? 1. The agreed entry must include the physical living arrangements for the child(ren) and a designation of which parent is the residential parent. In cases involving shared parenting, there must be a separate decree of shared parenting (if applicable) and a shared parenting plan which includes a statement that each parent shall be the residential parent when the children are in his or her care DR30(B)(3)(a). The shared parenting plan accompanying an agreed entry must contain a provision for decisions regarding school placement , and if it is necessary for school or public assistance purposes, a designation of legal custodian.DR30(B)(3)(b). The agreed entry must include a specific schedule of parenting time DR30(B)(3)(c). The agreed entry which incorporates the court=s standard parenting time schedule must be accompanied by a copy of the schedule DR30(B)(3)(c)(ii). The agreed entry must allocate the rights of parents to claim tax exemption for the child(ren) DR30(B)(3)(H). The agreed entry must include an order that any person who is the residential parent of a child, including any party to a shared ? 2. ? 3. ? 4. ? 5. ? 6. parenting decree and any other legal custodian shall send a notice of intent to relocate to the Domestic Relations Court compliance Office DR28(B)(3)(k). ? 7. SECTION III: CHILD SUPPORT - IF APPLICABLE ? 1. The agreed entry must include a statement of child obligation stated in dollars per month, a statement as to whether or not the support order conforms to the schedule of support contained in ORC 3119.021, and a state that such support, plus a 2% processing charge, is to be paid through the Butler County Child Support enforcement Agency or the Ohio Child Support Payment Center. All payments must be made payable in increments that coincide with the payor=s pay periods DR30(B)(3)(d). The agreed entry must include an effective date for child support to commence and/or terminate.
