The attached shared parenting plan is being returned because it is not in compliance with state law and/or local rule of court. The
documents submitted are defective for the reason(s) checked below, all of which are required by the Ohio Revised Code, Ohio Rules
of Civil Procedure, and/or local rule of court:
SECTION I: GENERAL

The plan must include both parties’ names, addresses, social security numbers, dates of birth and telephone numbers - if pro
se (DR 1 (BB)).
The plan must include the child(ren)s’ name(s), date(s) of birth, and social security number(s) (DR 1(B))
The plan must be accompanied by a IV-D application for child support services - if applicable (original copy only) (DR 9).
The plan must contain both attorneys’ signatures, addresses, phone numbers, and Supreme Court Registration Numbers
(Civil Rule 11).
The plan must contain both parties’ signatures.
SECTION II: PARENTING SCHEDULE - (if applicable)

The plan must contain a parenting agreement (DR 27 (B)(1)).
The plan must include a specific schedule of parenting time (DR 27 (B)(5)).
The plan which incorporates the court’s standard parenting time schedule was not accompanied by a copy of the schedule
(DR 610).
The plan must include language regarding advance notice by either party of their intent to relocate (DR 27 (B)(8)).
The plan must include language regarding equal access to the child(ren)s’ school or medical records or agreement of
limited access (DR 27 (B)(9)).
The plan must include language to keepers of records regarding their failure to comply with equal access (DR 27 (B)(9)).
SECTION III: CHILD SUPPORT - (if applicable)

The plan must include a support agreement and the amount per child and monthly obligation per child (DR 41 (B)(2)).
The plan must include an effective date for child support to commence.
The plan must be accompanied by a child support calculation sheet with both parties’ notarized signatures (DR 40(C)(1)) &
40 (D)).
The plan which deviates from guideline support must state the amount pursuant to the child support schedule.
The plan which deviates from guideline support must include a notice to compliance form (C-16) signed by a judge or
magistrate (DR 41 (B)(3)).
The plan must include a statement of the amount of arrears due the obligee and the state of Ohio.
The plan must indicate the date the arrears are owed through.
The plan must include the mandatory language included in (DR 41 (B)(4)(a)).
The plan must include the mandatory language included in (DR 41 (B)(4)(b)).
The plan must include language requiring payment of support through the Butler County Child Support Enforcement
Agency or the division of child support in the Department of Human Services, as appropriate, and language requiring
payment of 2% CSEA processing charge (DR 41(B)(2)).
The plan must include statutory language regarding gifts (DR 41 (B)(4)(c)).

The plan must include statutory language directing the parties to notify the CSEA in writing in the event of a change of
address or employment (DR 41 (B)(4)(d) & (e)).
The plan must indicate the method of payment to the CSEA (DR 41 (B)(5)).
The plan must include an order to seek work and a $20.00 per week child support order for the unemployed obligor (DR 41
(B)(5)).
The plan must include language directing the parties to notify the CSEA in writing of any change in the status of the child
which would terminate or modify the duty of support (DR 41 (B)(4)(f)).
SECTION IV: HEALTH INSURANCE - (if applicable)

SECTION V: MINIMUM CONTENT

A provision for decisions regarding school placement (DR 27 (B)(4)).
A provision allocating the rights of the parents to claim the child(ren) as tax exemptions (DR 27 (B)(7)).
